Abstract

Background:
Neonatal sepsis is a life threatening condition and a leading cause of neonatal morbidity and mortality globally. Early diagnosis is still difficult because clinical symptoms are often nonspecific and conventional blood culture methods are time-consuming. Recent advances in rapid molecular diagnostic technologies and biomarker-based approaches have improved timely detection and management of neonatal sepsis.

Objective:
To assess the effectiveness of rapid molecular diagnostic technologies and biomarkers in early detection of neonatal sepsis and their impact on clinical outcomes.

Methodology:
We conducted a prospective observational study in 220 neonates in neonatal intensive care units (NICU). Blood samples were analyzed by molecular diagnostics based on polymerase chain reaction (PCR) and biomarker assays (including C-reactive protein (CRP), procalcitonin (PCT), and interleukin-6 (IL-6). Diagnostic accuracy, detection time and treatment results were examined.

Findings:
Among the enrolled neonates, 68 (30.9%) had a diagnosis of confirmed sepsis. Rapid molecular diagnostics identified pathogens in 4.2 ± 1.1 hours compared to 48–72 hours for conventional cultures. The combined biomarker analysis identified sepsis with a sensitivity of 91.4% and a specificity of 88.7%. Early diagnosis reduced the average delay in starting antibiotics by 42% and survival was 94.1% among neonates diagnosed.

Conclusion:
The early diagnosis of neonatal sepsis is greatly facilitated by rapid molecular diagnostic technologies and the assessment of biomarkers. Including them in neonatal care can assist in early management, better survival outcome and decrease sepsis associated complications.
