Abstract

Background: Gestational diabetes mellitus (GDM) is the commonest metabolic disorder of pregnancy and is associated with substantial maternal, fetal and neonatal complications. Exposure to maternal hyperglycaemia during fetal life may have adverse effects on cardiovascular development, with an increased incidence of structural and functional cardiac abnormalities in the newborn. Understanding the association between gestational diabetes and neonatal cardiovascular outcomes is important to improve perinatal care and reduce long-term health risks.

Objective: To assess the relationship between gestational diabetes and perinatal complications and their effect on neonatal cardiovascular outcomes.

Methodology: A comprehensive review of recent clinical, observational and epidemiological studies was performed. A systematic review of the relevant literature on gestational diabetes, perinatal complications, fetal cardiac development, and neonatal cardiovascular health was conducted to identify the major risk factors and outcomes.
Findings: The evidence reviewed suggested that gestational diabetes was associated with a 30–40% increase in neonatal cardiovascular complications including myocardial hypertrophy, ventricular dysfunction and congenital heart abnormalities. The neonates born to mothers with GDM had about 25–35% higher rates of cardiac structural anomalies and a 20–28% increase in cardiovascular adaptation problems in the neonatal period. Early diagnosis and glycaemic control significantly reduced adverse outcomes.

Conclusion: Gestational diabetes has a significant influence on the cardiovascular health of the neonate and the risk of perinatal complications. Effective screening during pregnancy, management of glycemia, and neonatal cardiovascular monitoring are important to improve neonatal outcomes and reduce long-term cardiovascular risks.
