Abstract

Background - Erosive oral lichen planus (OLP) is a chronic immune-mediated inflammatory disorder that is characterised by painful red and ulcerative lesions that significantly impair oral function and reduce quality of life. Topical corticosteroids are the mainstay of treatment, but prolonged use is associated with adverse effects and relapses. Low-level laser therapy (LLLT) has been suggested as a possible non-invasive alternative because of its anti-inflammatory, analgesic and bio-stimulating properties.

Case Presentation: We present two female patients, 50 and 62 years old, with symptomatic erosive oral lichen planus with severe burning sensation (VAS 9/10). Both patients received initial treatment with standard pharmacologic therapy, including topical corticosteroids, antioxidants and supportive drugs, with only partial relief of symptoms. Low-level laser therapy (100 mW, 660 nm) was introduced weekly during the initial treatment phase, but it caused temporary symptom exacerbation and was stopped. Both patients received weekly 660 nm low-level laser therapy for 4 weeks, with topical anaesthetic, after 2 months recurrence, and complete clinical resolution with significant pain reduction and lesion eradication was achieved. No recurrence was observed at 7-month follow-up and the residual burning sensation was decreased to VAS 1/10 and 2/10 respectively.

Conclusion: The present cases show that 660 nm low level laser therapy is a safe and effective treatment option for erosive oral lichen planus with significant pain relief, faster lesions healing and longer remission period with no adverse effects.
