Acute Kidney Injury in Weil Disease: Cases Series

Leptospirosis is a zoonotic infection with a wide clinical spectrum, ranging from mild illness to severe disease with multi-organ involvement. Acute kidney injury (AKI) is one of the most common complications and may require renal replacement therapy (RRT) in severe cases. We report three cases of leptospirosis-associated AKI in patients with a history of exposure to contaminated water. All patients presented with fever, jaundice, myalgia, and oliguria, accompanied by laboratory findings consistent with AKI. Management included antibiotic therapy, intravenous fluid resuscitation, and supportive care. Diuretics were administered in cases with persistent oliguria. All patients demonstrated significant clinical and laboratory improvement, including recovery of renal function and normalization of urine output. None of the patients required renal replacement therapy. The duration of hospitalization ranged from 5 to 12 days. These cases highlight that early recognition and prompt supportive management may lead to favorable outcomes in leptospirosis-associated AKI without the need for renal replacement therapy.
