Abstract

Modern medicine is currently undergoing a paradigm shift from organ- and disease-oriented concepts to integrated approaches by linking molecular, clinical, therapeutic, technological, and population health domains. This shift is dictated by the fact that the majority of morbidity and mortality factors are complex traits and processes with multiple etiologies, outcomes, and mechanisms. Most diseases are determined by genetic, epigenetic, immune, metabolic, environmental, behavioral, aging-related, and socioeconomic individual factors, as well as their interactions. The burden of noncommunicable diseases increases due to persistent inflammation foci, while antimicrobial resistance, emerging pathogens, and the complexity of polyetiologic syndromes deteriorate the situation with global health security. This requires new approaches to understand the nature of the problems and offer integrated solutions. The present narrative review focuses on three central aspects of modern medicine and the current trends in basic science, clinical practice, and integrated health care strategies. First, the main mechanisms of disease include genomic, epigenetic, inflammatory, immunological, mitochondrial, metabolic, senescence-related, and microenvironmental processes as well as dysbiosis of the commensal microbiota. Second, the contemporary clinical practice increasingly emphasizes personalized and precision-oriented considerations, such as diagnosis and prognosis of disease using biomarkers, molecular mechanisms, and genetic variation, the development of therapeutic strategies, multidisciplinary care, disease prevention, and surveillance. Third, the new technological platforms such as AI, multi-omics, telemedicine, digital biomarkers, liquid biopsy, advanced imaging, gene editing, cell therapy, organoids, and learning health care systems can harness the opportunities of data science, including machine learning, for better health care. Nevertheless, there is a need for cautious adoption of these opportunities, with a particular emphasis on proper validation, implementation, surveillance, and regulation, as well as ethical, effective, equitable, affordable, and sustainable solutions within the context of human expertise.

Thus, medicine may enter a new era when the convergence of basic science and clinical practice guided by informatics and data science will transform health care through better detection, characterization, prediction, and management of disease while emphasizing prevention and patient-centered care.
