Abstract

This case study presents a 37-year-old female with a 12-year history of hyperten-sion, presenting with easy fatigability and breathlessness. Laboratory investigations revealed severe hypokalemia (K+ 1.3 mEq/L) and metabolic alkalosis (pH 7.67). Endocrine evaluation showed an elevated Aldosterone-Renin Ratio (ARR) of 7.4. Imaging confirmed a 20×18 mm right adrenal adenoma. The patient was diagnosed with Conn Syndrome and successfully managed medically before undergoing a cu-rative right adrenalectomy. This case underscores the critical importance of early ARR screening in young, treatment-resistant hypertensives and highlights the di-agnostic challenge when endocrine symptomology is masked by concurrent severe hematological derangements.
