Hypothyroidism and renal insuffi ciency: an overlooked relation?

Objective: Autoimmune thyroiditis is the most frequent form of thyroiditis and it constitutes the most prevalent cause of
goiter and hypothyroidism in countries where there is enough iodine intake through feeding. Thyroid hormones have a
well known role in kidney function. However, in current literature, there are few reports about this relation in children. Case
description: a 16 years old adolescent with symptoms suggestive of hypothyroidism. During the etiological investigation,
he was diagnosed with autoimmune chronic thyroiditis associated with renal insuffi ciency (hypercreatinemia and reduction
in glomerular fi ltration rate) and myopathy (with high values of creatinophosphokinase, oxalacetate transaminase and lactic
dehydrogenase). Replacement treatment with levothyroxin sodium allowed the resolution of symptoms with restoration of a euthyroid state, resolution of renal insuffi ciency and normalization of myolysis' markers. Comments: Hypothyroidism can
affect kidney function, leading to a decrease in glomerular fi ltration rate and consequent renal insuffi ciency. These effects
seem to be reversible, with a resolution of renal dysfunction after initiating replacement therapy with levothyroxin sodium.
