Objective: report the case of a teenager with secondary pleural effusion to Non- Hodgkin Lymphoma. Case description:
A male patient with 11 years, presenting cough, pain in the left hemithorax for 7 days associated with fever. In the physical
examination showed left hemitorax curved and swollen, with pain to palpation and auscultation with vesicular murmur
abolished. Was requested chest X-ray which showed pleural effusion on the left hemithorax followed by a thoracentesis which showed characteristics of exudate and cytological predominantly of lymphomononuclear. A new chest radiograph showed
a mass left hemithorax region. It was then held a chest tomography which revealed a mass in the anterior mediastinum, with
partially defi ned limits. A bone marrow biopsy and immunohistochemistry were performed and confi rmed the diagnosis of
lymphoblastic non-Hodgkin's T-cell lymphoma. Comments: The investigation of patients affected by mediastinal tumors
require detailed clinical history and physical examination. If there is suspicion of malignancy, pleural fl uid cytology is the
diagnostic tool of choice, with good sensitivity. In this scenario, the exudates associated with hematologic malignancies,
particularly Non- Hodgkin Lymphomas, constitute the real challenge in the differential diagnosis of lymphocytic pleural
effusions. In anterior mediastinal lymphoma, surgery is more important for the diagnosis, since these lesions respond
favorably to chemotherapy and/or radiotherapy.
