Translation and cultural adaptation of record data on rheumatic fever proposed by World Heart Federation

Abstract

Objective: The World Heart Federation (WHF) provides a network of information on rheumatic fever (RF), which is part
of a database in English for the record (DB-FR). The translation and adaptation to the Portuguese language would allow
the use in Brazil and other Portuguese speaking countries. This BD-FR allows the evaluation of diagnosis, the classifi cation
of RF, and the severity of some outcomes: completion of secondary prophylaxis, realization of heart surgery and death. In
Brazil we do not have a database on the frequency and the major outcomes of this disease. Methods: We used the method
recommended by the World Health Organization (WHO) for translation and cultural adaptation. The analysis of the group
turned to: 1) identifi cation of the patient, 2) diagnosis, 3) classifi cation of the rheumatic disease, 4) severity and 5) expected
outcomes. This process aimed to evaluate the relevance of various dimensions and capacity of the original instrument in
capturing them from the Brazilian reality. Results: The conceptual equivalence of the instrument is satisfactory and covers
the main dimensions of the FR in Brazil. About the semantic equivalence, a good agreement between the translators of both
phases (translation and version) has been observed.The overall meaning was considered altered, and the partial unaltered.
Some modifi cations were suggested to better adapt to our culture. We adapted the registration dates of diagnosis and
surgery. Conclusion: It is expected that this instrument can facilitate the registration of RF, improve the management of
patients and facilitate health planning.
